University of Ilorin Teaching Hospital, UITH School of Health Information Technology (SHIM) admission form is out for the 2016/2017 academic session. Applications are open to candidates into the National Diploma (ND) Programme both Full-time and Part-time studies as well as those for a Higher National Diploma (HND) of UITH SHIM for the 2016/2017 academic session.
What is the Cut-Off Mark For Admission 2016/2017
The Cut-off mark that must have been attained by any candidate for the UITH SHIM admission for the 2016/2017 academic session is 150 and above.
Admission Requirements For UITH SHIM 2016/2017
These are the admission requirements for UITH School of Health Information Management for the ND (part-time and full-time) and the HND studies;
1. UTME results which is required in any three subjects from Mathematics, Biology, Physics, Chemistry, Geography, Economics, Yoruba, Government and Religious Knowledge.
2. Candidates should have their West African School Certificate/General Certificate of Education/SSCE/NABTEB with 5 passes at credit level in five (5) papers obtained at not more than two (2) sittings, must include English Language, Mathematics, Biology or Health Sciences and any two Chemistry, Physics, Economics, Government, Geography, Religious Knowledge and any Nigerian language.
3. Applicant applying for Part-time must attach the appointment letter from his/her working place.
4. applicants for the Higher National Diploma must posses National Diploma Certificate or Health Technician Certificate.

To apply for admission into UITH SHIM programme for the 2016/2017 academic session;

2. Proceed to pay a sum of N5,000 into the UITH SCHOOL COMPLEX REVOLVING FUND ACCOUNT through remita.
Application closing Date 2016/2017`
The closing date for the application of UITH SHIM admission for the 2016/2017 academic session is on or before 30th June, 2016, as all completed application forms with original receipt of payment must reach the office of the Registrar.
Any candidate with blurred or unclear passport photograph on the form will not be admitted for the screening exercise as the photograph will be used as a means of identification.Any candidate who fails to present himself or herself for the screening exercise as scheduled will not be considered for admission.
Screening Date 2016/2017
The date for the written examination is 12th July, 2016, and the screening/interview date is 13th July, 2016 at the University of Ilorin Teaching Hospital School Complex(Former Maternity Hospital) Amilegbe, Ilorin.
